Prescription Management Service

We offer a full prescription management service where all prescriptions are dealt with by our Prescription Team with very little input from onsite clinicians or each individual element can be utilised alone.

Prescription Clerk – Our prescription clerks are qualified Level 2 Pharmacy Dispensing Assistants who have a very good foundation knowledge of the safely handling and processing of prescription medications.

Pharmacy Technician - All requests for prescriptions submitted via ‘requests with query’ will be dealt with by our trained technicians. This includes all of the queries processed by the Prescription Clerk but needing further review along with any other prescription queries such as changes in dose, direction, quantity and out of stock alternatives etc.

Prescribing Pharmacist – Our prescribing pharmacist all have extensive experience of working in Primary Care and the challengers that prescribers face. We schedule our Prescribers to log on to sites usually later in the day when all of the preliminary work has been completed by the Prescription

Document Management

Clinical Coder - We provide highly experienced Clinical Coders to process your correspondence in an efficient and timely manner. This includes ensuring all of the appropriate diagnosis and observations are accurately coded in the patients record before filing or forwarding to a clinician if further action is required.

Pharmacy Technician - Our Pharmacy Technicians are all highly skilled professionals, proficient at safely transcribing changes to treatment plans detailed in hospital discharges and outpatient letters . This helps free up Clinical Pharmacist time for more clinical work and also leave more time for GP’s to concentrate on their patients. Any actions needing a senior practice clinician review are tasked directly to them.

Who are primary care support?

Structured Medication Reviews (SMR)

Our clinical pharmacists are experts at carrying out in depth SMR’s with a wide range.

As an alternative, cost effective way of working, we often task our technicians with the data collection aspect of the SMR process. They will liaise with the care home regarding any issues they are having along with cross referencing the patients MAR chart with your clinical records looking for any discrepancies. They will link medication to medication, check routine monitoring has been completed (and schedule if outstanding), check compliance along with general tidying up the patient medication page. ACB scores will be calculated along with risk of GI bleed and AKI. All of this information will be coded on the patients medication record whilst using the appropriate templates.

In addition to the standard SMR data collection we also discuss any mobility issues (risk of falls) eating habits and any other problems they may be having with the patient.

Once the data is collated the populated template is forward to the Clinical Pharmacist or GP for the final clinical assessments which will then ensure they are completed by a prescribing clinical therefore meeting the DES requirements.

Polypharmacy Medication Reviews

Polypharmacy reviews are particularly useful when it comes to trying to manage cohorts of very complex patients. Our skilled pharmacists often have more time to spend getting to the bottom of some of the challenges these patients are having, which in the long term helps to reduce the amount of queries and issues landing on the clinician’s desk.

General Medication Reviews

Patients using repeat medication are recommended to have an annual medication review, but those who have more complex health needs often need them more frequently.

Our Pharmacy Technicians have been upskilled to be proficient at Level 2 Medication Reviews for patient using up to 3 or 4 mediations.

Our Clinical Pharmacists have extensive experience at carrying out a wide range of medication reviews from Long Term Condition reviews to NOAC and Mental Health reviews.

Electric Repeat Dispensing (eRD)

Electronic repeat dispensing is a brilliant idea to help reduce the amount of repeat medication requests per month, if introduced correctly. Simple, established patients can be safely transferred over to this system and then will not need to submit another request to the surgery for up to 12 months when they are due their next review.

Extended Access

We are excited to be able to offer a service to support practices and PCNs with the challenging requirements of the Extended and Enhanced Access service.

We have a number of experienced primary care pharmacists who are highly skilled and available to work weekday evenings (until 8:30pm) Saturday and Sunday, carrying out remote consultations. These appointments can be made for Structured Medication Reviews, Polypharmacy Reviews or standard Medication Reviews and will all be carried out using the appropriate template within the practices clinical system. These consultations ensure a good skill mix in the extended access hours appointments.

Prescription Signing

Our prescribing clinicians can sign all of your prescriptions in a timely manner meaning patients get the quickest service available and your clinicians are free of another end of the day burden. Our prescribers do not bulk sign but will go though individual requests ensuring the items have had the appropriate monitoring completed and the item is safe to issue.

We can manage the whole prescription process or just check and sign approved prescriptions. The choice is yours.

High Risk / Shared Care Drug Management

With increasing scrutiny in the management of patient using these high risk drugs, we have created robust systems that help ensure patients routine monitoring is proactively managed.

This not only helps to ensure patients are taking their medication safely but also helps to reduce the inevitable increased work load that will follow once the essential monitoring have been missed.

Patients using medications that are only recommended to be prescribed in Primary Care under a Shared Care Agreement, are often transferred to our care without the appropriate paperwork in place.

Not only is this paperwork vital to ensure the prescribing clinician has clear prescribing guidelines, it also ensures the practice can receive payment for their part in the Shared Care Agreement.

Our team of Pharmacy Technicians can obtain the appropriate paperwork, coordinate the patients routine monitoring at the appropriate intervals and also support the claiming process.

We offer a variety of medicines manage support that can be tailor to suit the individual needs of a practice or a PCN. This support can be anything from prescribing audits to incentive scheme workstreams helping to maximise potential reimbursements.
